Completing Your Application to the
Graduate Program in Communication Sciences and Disorders University of Minnesota Duluth

1. What do I need to submit to apply?
A completed application consists of the following components:
- University of Minnesota Graduate School Application
- Undergraduate transcripts
- Three (3) letters of recommendation; two of which must be from professors familiar with your academic performance and your potential to succeed in graduate school. Letters of recommendation are to be submitted using the Applicant Recommendation Form.
- A writing sample in the form of a personal statement that addresses your short term and long term academic and professional goals, why you are interested in pursuing a graduate degree in Speech Language Pathology, and why you are interested in pursuing your degree at UMD.

2. How do I submit each of these application components?
- University of Minnesota Graduate Application: Application to the University of Minnesota Graduate School is done online using ApplyYourself. Use the Apply Yourself link to direct you to the website to get you started.
- Undergraduate Transcripts: College transcripts must be sent to the University of Minnesota Twin Cities campus. The address is Graduate School, 309 Johnston Hall, 101 Pleasant St. S.E., Minneapolis, MN 55455. If you are also applying to the CSD program in the Twin Cities, you must provide a separate transcript to BOTH programs. That’s right, two transcripts sent to the same place.
- Letters of Recommendation: It’s very important that your recommendation providers use the Applicant Recommendation Form for their recommendation. You can download the Applicant Recommendation Form here and electronically send this form to each of your recommendation providers. Remember, this is a REQUIRED FORM. Applications missing the Applicant Recommendation Form will be considered incomplete and WILL NOT be reviewed. Additionally, Apply Yourself will alert your recommendation providers to the use of this form with your application and give them an opportunity to download it themselves, with instructions on how to complete and submit the form. It is important that your recommendation providers click on the “special instructions Duluth campus programs” from their Apply Yourself link for recommendation providers.
- Personal Statement: Prepare your personal statement using any word processing application (i.e. Microsoft Word). In Apply Yourself, click “Graduate Program Supplemental Information”. Upload your personal statement under “writing sample”.
- Checklist of Academic and Clinical Undergraduate Experiences: A Microsoft Word version of the checklist is available here. Download the form and complete. Save the completed checklist. In Apply Yourself, click “Graduate Program Supplemental Information”. Upload the checklist under “Graduate Program Additional Application”.

3. How can I keep track of my application?
Apply Yourself will allow you to keep tabs of ALL parts of your application. It would be a good idea for you to check on the progress of your application in ApplyYourself periodically to ensure your application is complete and submitted.
4. Will the department notify me when my application is complete?
No. You are responsible for making sure that all components of your application are complete and submitted by the application deadline (Monday, February 1, 2010).
